Reporting Alleged Abuse

This report should be completed by the Site Manager, Human Resources Manager or designate and must be submitted within seven (7) days of the employer’s notification to the employee of the suspension pending investigation or termination. A copy of this report must also be provided to the employee and the union designate (if represented by a union).

Under agreements with the Ministry and between Health Authorities and service providers, all publicly-funded healthcare providers who employ Health Care Assistants are required to employ only individuals who are registered with the Registry.


In order to fulfill its mandate, and with the consent of the applicant, the Registry collects uses and discloses an applicant’s personal information for: the purpose of registration, or a use consistent with the purpose of maintenance of registration; and/or suspension of registration; and/or reinstatement of registration.


  • 1. It is a contractual responsibility for Employers that are publicly funded to report to the Registry any Health Care Assistant alleged abuse to a patient, client or resident.
  • 2. As a matter of contract, Employers will be required to ensure that all Health Care Assistants are currently registered with the Registry before they are offered employment.
